  • The student’s full legal name, exactly as registered at their school.
  • The full name of the school the student attends.
  • The student’s grade level for the 2026-27 academic session.

Step 2: Complete the Online Application Form

Parents with multiple eligible children can submit all their children’s information on a single application form. The form is available on the official government portal:
